FRACTURES/SEPARATIONS
Cause
Fractures of the humerus, scapula and clavicle often result from a direct blow or a fall onto an outstretched hand. AC separations usually result from a fall onto the point of the shoulder.
Treatment
Most are not treated with surgery. Treatment usually consists of a sling for comfort. Range of motion exercises start as soon as acute pain resolves, within two weeks. Early mobilization is important to avoid loss of range of motion, possibly leading to frozen shoulder.
Surgery can stabilize the bone with plates, screws, or pins. Partial shoulder joint replacement (hemiarthroplasty) is rarely required. AC joint separations are initially treated emphasizing an exercise program. Surgery is possible later, if pain and loss of function persist.
